I consent to Zenith Insurance Company (“Zenith”) collecting, using, and storing my payment information (“Information”) to process premium payments for my insurance policy. Information includes credit card number, credit card security code, my name on the credit card, and credit card expiration date. I also consent to third parties acting on Zenith’s behalf for this purpose, and to my Information being provided to Zenith’s credit card processing company. The collection, use and storage of my Information is subject to Zenith’s Privacy Policy. Zenith shall not collect, use or store my Information for any other purpose, or disclose my Information to any other party, without first obtaining my consent, except where required by law. This consent to collect, use, store, or disclose my Information shall expire upon the expiration or termination of my insurance policy with Zenith.
